CONSENT FORM FOR PHOTOGRAPHS

I, the undersigned parent, consent to my child(ren) attending Joy Homeschool Co-op for any JHC function being photographed for the use in print, advertisement, social media on JHC website, or video for promotion of JHC. I agree that JHC shall have the right, but not the obligation to use my child’s photograph, likeness (including caricature), for their website at any time and for any other purpose or materials the JHC deems necessary. The child’s name will not be used with the photos.
